I knew the racist spade connotation but don't think I've ever heard someone use it that way for reals. I wasn't sure if "call a spade" was racist.

Which is why we had a blanket rule for the improvisers (trust me, it wasn't the comp sci people I was doing it with) that you were not allowed to tell strangers why beforehand, and sometimes not even afterwards.

I hated both games (why should I tell you shit unless I want to tell you shit, otherwise it is none of your damn business) so if a game started, I flat out stated that nothing that coming out of my mouth was going to be truthful. And proceeded to lie. Usually pretty boring or outlandish lies. It always confused people. But I rarely was coerced into a game again.

I never played drinking games or truth-telling games, not even when I was at the hard-partying, all-but-orgy-having radio station in college. I'm thinking we didn't bother with excuses to get drunk, and we were too busy racking up interesting achievements to be tossing them out in games.
