Muscle spasms hurt a lot, mostly because when the tightened muscles move around, it kind of starts over... the other pain I have a lot of experience with is abdominal, which doesn't feel the same at all, but can hurt a lot.

Appendcitis was pretty bad, but I know that was near a 10 because it didn't burst. That's MUCH worse from everything I've heard.
Some of my recurring chronic leg/foot pains are bad. Basically arthiritis in it's early stages (and a lifetime of flat feet). They mainly cause problems (aside from with walking) at night, because they keep me awake, and feel like they hurt worse.
I have had some headaches (some hangovers, some not) that made me feel that ripping my eyeballs would be prefarable to the pain I was feeling.

The most pain I've experienced was when a doctor swabbed my sinuses with liquid cocaine prior to running tubes up my nose. Right after that they put me under for surgery. I wondered why they couldn't have done the swabbing after they put me under.
Oh, yeah, getting my acne cysts injected when I was a teenager was pretty bad, too.
Things going up your nose are incredibly discomforting even if they don't "hurt" per se. I had a nose cam a couple of times last year when I was sent to an ENT specialist, and it was the most uncomfortable feeling I've ever had.

My tattoo was painful, but only while the needle was active. When the inking stopped, the pain stopped. So that was cool.
The most pain I've been in was when (whitefonted for pretty gross TMI) a bit of tissue was caught in my cervix after a D&C. Bearable only in the sense that I didn't actually die. I wasn't given a pain-killer until after removal. Paying that ER bill made me really angry.
