I finally got to watch last night. That scene and that line/delivery, in particular, and Dean's reaction to it - riveting. I was backing up a little into my couch there.
Really liked the ep. I don't think Sam has been sleeping with Ruby. Is she jumping from body to body quickly? Is this so as not to kill/permanently scar the humans? Wasn't there some discussion about length of time in possession being the deciding factor about how damaged the human gets?
On the other hand, how psyched must Castiel's accountant vessel be in there? "I'm doing God a solid, and this Castiel dude is FUCKING AWESOME! Damn right, '...show me some respect.'" Hell, even if he ends up shuffling off this mortal coil, I imagine he is anticipating his eternal reward with glee.

Sam said, "Yes," in a "Have you met me?" kind of way. I know the final shot was of Dean staring intently into space while his brain was spinning cogs and wheels furiously. I don't remember what happened between those two moments, though.
