Looks like a shadow to me, but I didn't see that shot on TV. I just saw a face shot when she was lying there, with her eyes open, but unable to move or respond.
Since this whole superpowers thing, according to Mohinder, is based on "evolution" and "genetic markers," that's where it's assumed to have come from. My theory is that it's just a marker the series is using, and there won't be any real explanation for it. It just is.
This is why I don't pay attention to symbols unless/until they have the characters discuss the symbol.
I'm also in the "it looks like shadow" to me camp. Considering the show hasn't been shy about blood and gore, I think if Niki was bleeding, we would have been able to see it clearly.
