I don't think there's any reason to assume that tastes outside societal norms require more complex motivations than tastes within them.
Yeah, maybe not. I mean, most people's reason for doing anything is probably "because I want to." I know that the teacher at my old job who had big disks in her ears and lip (edit: and tattoos on her face. heh -- I forgot about that) had reasons beyond "it's pretty," but it's not a requirement or anything.
I can't speak for anyone else, even on if tattoos are more complex than multiple ear piercings, but the sole reasons for getting mine were "I want" and "pretty".

I see what you mean, but I keep thinking the more complex the act and its consequences, the more possible the motives were more complex.

Well, my comment was made about the more extreme sort like the picture you linked, and really, I think Jilli gave an answer. It just wasn't posted when I was posting my question.
In the picture you linked, that man's whole back was covered in piercings. It's got to be hard to keep them clean. It's got to be uncomfortable to sit and lie in certain positions. It's likely that they could snag on clothing. He probably can't reach all the piercings, himself. It would cost more to do. He can't really see it without the aid of mirrors.

Emmett's fever has subsided. We did a walkabout in the neighborhood, scoring a popgun, a Hot Wheels motorcycle, a finger splint for him, a new 3 Stooges DVD, Mirrormask DVD and I got
stuff to clean out my ears since I've been going deaf over the last couple days. And I flushed a small animal out of my head and it was gross but now I can hear again, huzzah.

Well, once Jilli indicated it was most likely temporary, the issue really went out the window, but I didn't not want to answer the questions. I can see the discomfort or pain being a turn on for a limited amount of time, but not living with discomfort on any sort of extended basis.
