Regarding Cindy, I read that she was the flight attendant who gave Jack the extra bottle of booze. I personally don't remember that, but it could work...
That would make sense. I'd have to watch the pilot again.
The scenes of Jack that are intercut with Eko asking about the doctor look suspiciously like he's reacting to someone with a weapon, not a patient -- his reaction to that tends to be leaping up, having the person put on a flat surface, etc. Though it's possible they really are from the Eko scene and he's just reacting to the fact that Eko is a stranger.
