I don't think there was ever an uninitentional, but there's been immortals decapitated by trains and chopped up in boat engines that have produced quickenings.
Unrelated, but I also seem to recall we also saw Amanda shove Duncan out of the way so she could get the Quickening from an immortal he just killed so it's definitely a whoever is standing closest and in-range thing.
