Yeah, that annoyed me too. Or would have, if I hadn't seen the series. So it's a sympathetic annoyment for the new viewers.

I'd guess from the same place they got "Hoban" and "Inara" (provided none of them are pre-existing names, which any of them easily could be): they did what I've done when writing fictional 'verses and kept a list of good names thought of out of nowhere for later use. I still have my list somewhere, and I'm looking forward to getting to use a few.
Also: Wiki says the show takes place in 2517. Is this canon or did someone just pull the number out of his/her ass and it somehow became accepted?
The River picture spoilage was one of Joss' primary beefs with Fox over the Firefly promotion campaign. Youda thunk SciFi would have learned from their mistake.
Not only that, but they put the image of River in the promos, and then didn't show the pilot. What's up with that?
I know an Inara--she's on one of my writing lists. I haven't gotten around to asking her where her name comes from, though I do know it's her real name and not a case of a Firefly fan taking it as a screen name.

I seem to recall that year displayed on the screen in the voiceover intro "Here's how it is...".
The chyron did indeed read "2517 A.D." in the original intros.
