Apparently Paget Brewster never wanted to come back this season but had no choice after her comedy pilot wasn't picked up because of the way her contract read. This is according to TVLine, which I can't link to on my phone.

Hey, what if Misha comes back, and it's not as Cas? Or he never remembers he's Cas at all, and then he dies anyway? That would be so FUCKING TORTUROUS.

Which means it's on the table.

My current understanding that is tainted with fic I don't know how much...Sam has to go into the mental hospital because he has a breakdown and they find a guy that looks like Cas there.

Am I making shit up? I mean, I haven't read that story, but I don't remember reading that article either.

Well, there's a picture of Misha (as a character on a show, presumably, although...) sitting on an institutional-looking bed in what looks like hospital clothes out there somewhere. (Did you link it? sumi?), so your hypothesis does not sound unreasonable to me.

That would be fucked up! I'd kind of like to see it.
