It appears that the one who pulled Sam out of hell (minus his soul) will be revealed, according to Jared at Jus in Bello 2. Given Castiel's resistance to getting Sam's soul back, I have to wonder if Castiel was behind it (and perhaps lacked the mojo or the bargaining power to get his soul too).
Really huge Supernatural spoiler: The brotherly bond will be back in full effect 6x22. More than now? I don't know. It's pretty solid currently. Also, season cliffhanger.
In the Castiel-centric episode Bobby is supposed to be telling Dean the truth about Cas and Dean doesn't want to believe it. Oh, woes. Regrettable things indeed.

Sort of? Although they are less so than most prime time series, there's a whole dual or triple storyline going on, bullying, out vs not and the drive for popularity, so I think it plays into all of them. There's been a huge shitstorm because the source of this was an extra and rumor has it that Ryan Murphy is trying to get Fox to allow him to reshoot or at least put that out there as a foiler (or maybe the whole story is a plant to keep the plot alive?)
